Neighbor to Neighbor Program, 12:30 p.m.
The Neighbor to Neighbor Program trains and provides volunteers to serve as a caring partner to help an older adult who is transitioning from a hospital in-patient stay to remain at home and in their community. The Neighbor to Neighbor volunteer will help these older, often frail adults by providing assistance, guidance and advocacy with non-medical, post-discharge tasks that are needed by a newly discharged individual, when they return to their home.
Started last May, the program is in need of additional volunteers. Training will be held in late February. The Neighbor to Neighbor initiative is supported by several partners, including local volunteer organizations, offices for the aging, hospitals and NY Connects. The program is administered locally by Community Caregivers, Inc. and the Capital Region RSVP Program.
